1. FORM TO REQUEST SPACE
Before signing this form, please read items 1-5 below: all-in-one Document.
2. CHECKLIST: Before, During, and After Your Meeting
Group Leader must share these items with all participants; everyone must comply fully.
3. ATTENDANCE SHEET: Please choose one:
- Online, Realm: Attendance taken in Realm is automatically updated with contact information if Participants are UUCD Members or Friends.
Leader needs Realm login (contact office for help).
. - Online ATTENDANCE FORM: submitted to office@uudavis.org when you hit “Submit.” No login needed.
. - Paper: Attendance Sheet PDF: Leader can keep the completed list or put it in the USPS Mailbox delivery slot on their way off site.
