  • UUCD General Workplace Safety (Training materials)
    • Map: Evacuation Routes
    • Map: Emergency Tools Bridgehouse; Main Building, Library/Office, Cottage
    • Emergency Plans (Who to call: posted in each building.)
      • Supervisor, if available.
      • In case of serious or life-threatening injury, call 911 immediately.
      • If help is needed for a non-life-threatening injury: Nurse Hotline

Reports (Forms: All Reports will be compiled by Admin)

• Monthly Inspection of Workplace Violence Hazards
  (Completed by Facilities Coordinator)
• Facilities Repair Request/Potential Hazard Identification
  Employee notifies church office of facility or person potentially posing danger
• Hazard Assessment and Correction Record
  (Completed by Facilities Coordinator)
• Accident/Exposure Investigation Report
  Supervisor & Employee complete following Accident/Exposure
• Workplace Violence Incident Report
  Supervisor and/or Employee complete following Workplace Violence
